2016 proved to be an outstanding year for the partner companies as they continue to achieve positive financial results while continuing to deliver the highest level of care. The partner companies have led the way in Hackensack Meridian Health’s expansion of programs and services outside of the traditional Monmouth and Ocean counties’ markets with many services already operating in other counties.
As an example, Hackensack Meridian Health At Home, the largest provider of home health care services in Monmouth and Ocean counties, has partnered with JFK Health to expand its network of in-home health services to patients and families in Middlesex, Somerset and Union Counties. Since our merger in 2016, JFK At Home has experienced increased referral volume by obtaining cases from Raritan Bay Medical Center. Hackensack Meridian Health also has a service partnership with Alert Ambulance Service. With more than 100 vehicles, Alert is the largest regional ambulance company in New Jersey, providing approximately 78,000 transports annually. The company has experienced growth in 2016, much of it from assuming responsibility for ambulance services at Raritan Bay Medical Center. In addition, Alert has also recently started providing ambulance services at Palisades Medical Center and Mountainside Medical Center.
The merger with Raritan Bay Medical Center and Hackensack University Health Network provides these partners with a broad continuum of services and the opportunity for exponential growth in new markets.
Helping Our Communities Thrive
Hackensack Meridian Health At Home has developed and launched the Safe at Home Memory Care program, the first of its kind in the region, to assist patient and caregivers from the earliest stages of diagnosis and evaluation, through the many phases of memory care planning and management. The program has trained 68 nurses, 101 CHHAs, and 22 therapists in Dementia Capable Care. This training focuses on what people with Alzheimer’s and related dementias (ADRD) can still do instead of what they can’t do. This training empowers our organization to provide unsurpassed person-centered care that helps persons with ADRD to thrive, not just survive.
Coming Soon to a Community Near You: The Villas at Manalapan
In June 2016, financing was secured for construction of a new senior living community in Manalapan. The Villas at Manalapan, an upscale vibrant boutique assisted living community set on seven acres off of Gordons Corner Road, is expected to launch in March 2018. The Villas at Manalapan is uniquely focused on our Revite Wellness Health Program focused on prevention, wellness and the reversal of chronic disease. Personalized health and wellness plans, including individualized nutrition and fitness programs, a full spa and dedicated health program will be developed for all residents.
The 100,000-square-foot facility will house 96 residences including independent living apartments and assisted living and memory care residences, and will offer luxurious amenities beyond what most senior living communities offer to ensure an extraordinary experience. The facility will include gourmet fine dining with an open view into the kitchen serving fresh and seasonal meals, a full lounge and cocktail bar, fitness center, beauty salon, full spa, a cinema theater and a great room to enjoy musical concerts, performing arts and lectures. Outdoors, residents can enjoy beautiful gardens, walking paths, patios and an outdoor fire pit. It is our belief that if you feel well, you can live well, and we are excited to be developing these new residences for the community.
Nursing & Rehab Facilities Receive Highest Rating from U.S. News & World Report
Hackensack Meridian Health Nursing & Rehab facilities at Brick, Shrewsbury, Wall, Ocean Grove and Bayshore in Holmdel, along with The Harborage in North Bergen, received the highest overall rating of five stars in U.S. News & World Report’s eighth annual Best Nursing Homes. These five-star ratings underscore the high-quality care we consistently deliver to our patients across New Jersey.
28 Practices Participating in Historic Partnership to Strengthen Primary Care
Hackensack Meridian Health Medical Group had 28 practices out of the more than 2,900 primary care practices nationwide participating in Comprehensive Primary Care Plus (CPC+), a partnership between payor partners from the Centers for Medicare & Medicaid Services (CMS), state Medicaid agencies, commercial health plans, self-insured businesses and primary care providers. This partnership is designed to provide improved access to quality health care at lower costs.
Hackensack Meridian Health Medical Group started on the journey to transform our primary care practices to Patient Centered Medical Homes (PCMHs) more than three years ago, and we have achieved NCQA Level 3 PCMH recognition for 29 primary care practices at this time. Our PCMH efforts prepared us for CPC+ and we are very excited about this partnership that will support our continued focus of providing quality care to our patients.
Through CPC+, CMS will pay primary care practices a care management fee, initially set at an average of $15 per beneficiary per month in Track 1 and $28 per beneficiary per month in Track 2, to support enhanced, coordinated services on behalf of Medicare fee-for-service beneficiaries. Simultaneously participating commercial, state and other federal insurance plans are also offering enhanced payment to primary care practices designed to support them in providing high-quality primary care on behalf of their members.
For patients, this means that physicians may offer longer and more flexible hours; use electronic health records; coordinate care with patients’ other health care providers; better engage patients and caregivers in managing their own care; and provide individualized, enhanced care for patients living with multiple chronic diseases and higher needs.
The five-year model started on January 1, 2017, with CMS soliciting a diverse pool of commercial health plans, state Medicaid agencies, and self-insured businesses to work alongside Medicare to support comprehensive primary care.
New Center Provides Unique Health and Wellness Therapies to the Community
Hackensack Meridian Health formally launched its Integrative Health & Medicine program in Monmouth, Ocean and Middlesex counties last July with the opening of the Center for Integrative Health & Medicine at Hackensack Meridian Health Village at Jackson. The new center and program empowers patients to reach the highest expression of themselves, engaging their mind, body and spirit to achieve optimal health and improved quality of life.The Hackensack Meridian Center for Integrative Health & Medicine couples traditional clinical medicine with healing therapies, treating the whole patient and not just their illness or disease. It is the only integrative health program to be built upon five Pillars of Wellness – Sleep, Activity, Purpose, Nutrition and Resilience.At the Center for Integrative Health & Medicine, patients can address their health concerns and goals, and receive a customized health and wellness plan developed by the center’s integrative specialists. The center’s integrated health physician, health coaches, nutritionist, health psychologist, acupuncturist and other professionals work with the patient and their specialty physicians to help each patient achieve their health and wellness goals.The opening of the center demonstrates our dedication to a comprehensive, patient-centered approach to health care. Providing the community with these unique, innovative services will empower them to take control of their health.The creation of this program in Monmouth, Ocean and Middlesex counties was made possible thanks to philanthropic members of the community. Their thoughtful contributions will greatly impact the way health care is delivered in New Jersey.
Hackensack Meridian Health Village Receives Prestigious Economic Development Award
The award was presented at the Monmouth – Ocean Development Council’s (MODC) 51st Annual Silver Gull Awards Dinner. Since its opening in 2014, Hackensack Meridian Health Village at Jackson has not only improved access to health care and services, it’s also positively impacted the area by creating new jobs, and serving as an anchor for overall economic development.
